Trump's Plan for ACA Subsidies: Direct Payments to Americans? Dr. Oz Explains (2025)

The Trump administration is considering all possible avenues regarding the impending expiration of subsidies under the Affordable Care Act (ACA), according to Dr. Mehmet Oz, the administrator for the Centers for Medicare and Medicaid Services (CMS).

In an interview with CNN's Dana Bash on 'State of the Union', Oz emphasized that the president is open to all options, but expressed concerns about the subsidies' implementation during the COVID-19 era. He stated, 'Our primary goal is to ensure that people have access to healthcare coverage.'

The subsidies, which were introduced during the pandemic and extended by the 2022 Inflation Reduction Act, are set to expire at the end of the year. House Democrats have proposed a discharge petition to extend the subsidies for three years, while Senate Majority Leader John Thune (R-S.D.) has committed to a December vote on the credits in the Senate.

President Trump has suggested an alternative approach, proposing to send direct payments to Americans instead of insurers. In a Truth Social post, he claimed that Americans can 'PURCHASE THEIR OWN, MUCH BETTER, HEALTHCARE, and have money left over' if they receive the credits directly. Trump's proposal implies a potential termination of the ACA.

Dr. Oz highlighted the healthcare legislation's 'fundamental flaws', citing concerns about waste, fraud, and abuse. He mentioned that these issues are primarily caused by agents, brokers, and third parties enrolling individuals in the ACA marketplace or altering their plans without their consent. According to the KFF, a health policy research group, these practices are paralyzing the system.

Oz suggested that addressing fraud, waste, and abuse could lead to discussions about extending subsidies. He believes that there are ways to utilize funds more efficiently by providing direct payments to Americans. If the subsidies are not renewed, millions of individuals will face increased premiums next year, with the average ACA marketplace premium for the lowest-cost plan projected to rise by $13 in 2026, according to CMS.
